PHP aftercare planning reviews progress, remaining needs, warning signs, and follow-up responsibilities before the program ends.

Because PHP is time-limited, a structured transition plan helps carry gains into IOP, outpatient therapy, psychiatry, recovery support, or other recommended services.

With appropriate authorization, Waterview may coordinate with outside providers and authorized supports so the PHP discharge plan is practical and understood.

PHP is time-limited, so transition planning starts early rather than waiting until discharge. The team reviews progress, remaining symptoms, safety, practical barriers, and the amount of structure still needed. These discussions help determine whether the next step may be IOP, standard outpatient care, psychiatry, recovery support, or another clinically appropriate service.
